About how long will my office visit take?

It depends on the reason for the visit. To allocate the appropriate length of time for your appointment, my staff will ask about the nature of your visit when you call for your appointment. In this manner, you may be assured of sufficient time to discuss your concerns. However, due to the nature of my practice, occasionally there may be delays during appointments.

Who will I see or talk to if my doctor is not available?

Dr. Fogwell usually will see or talk with you during normal office hours. However, when he is in surgery or away from his office; you may be assured that our nurse practitioner Cristi, or Dr. Hunt’s or Dr. Dullye’s office will return your call.

Where is the parking for your office?

There is parking behind the Margot Perot Women’s and Children’s Hospital building. There is underground parking, reached by the top lot.

What are your normal office hours?

Dr. Fogwell’s office is closed on Mondays and take lunch daily from 12:00-1:00 p.m.

How can I pay for your services?

We recognize the need for a definite understanding between patient and doctor regarding financial arrangements for medical and surgical care. The responsibility for payment of these fees is your direct obligation. Because of the high cost of billing, we request payment when you are seen. You may pay by Visa, MasterCard, Discover, American Express, check, or cash. For the insurance plans for which we are a preferred provider, we will collect co-payment amounts at the time of service. You will also be responsible for any coinsurance or deductible amounts that may be due. Please note that because of the large number of plans with which we must work and the fact that each plan is different, it is your responsibility to determine whether or not you need to obtain a referral from your primary care physician.

You will be billed if we do not receive payment from your insurance company. If the payment from the insurance company is less than the fee, you will be responsible for prompt payment of the balance. If the payment from the insurance company exceeds the stated fee, a check for the excess amount will be mailed to you promptly.

What happens if I cannot make my appointment?

If you are unable to keep your scheduled appointment, we ask that you contact us as soon as possible to reschedule or cancel so that other patients may use this time. For those who do not contact us prior to missing a previously scheduled appointment, we reserve the right to charge for an office visit. Please be aware that insurance providers will not cover this expense.

Can I bring my children with me to my office visit?

Although we love your children, because of the high incidence of contagious infection in youngsters, we request that no children be brought to the office.

Does Dr. Fogwell’s office charge to complete disability forms?

Dr. Fogwell’s Office provides one complimentary Disability, FLMA, Prior Authorization, Leave of Absence or Insurance form completion. The normal turn-around time for these forms is 7-10 business days.

If more than one form is requested or if changes are requested after the first form has been completed there will be a $25.00 charge per form.

If the disability form is needed earlier than seven days, there will be a $25.00 rush charge.
